JOB DESCRIPTION

JOB TITLE:  PROJECT COORDINATOR - CASE MANAGEMENT
JUVENILE REDEPLOY SERVICES

GRADE:         19, non-union
SALARY:      $75,704
LOCATION: 1100 SOUTH HAMILTON AVE., CHICAGO, ILLINOIS AND OTHER            COURT FACILITIES THROUGHOUT COOK COUNTY


The Redeploy Juvenile Project Coordinator- Case Manager is an employee of the Circuit Court of Cook County and reports directly to the Director of Juvenile Redeploy Services or their designee.  

The Redeploy Juvenile Project Coordinator- Case Manager:
  • Is available to the Court and the parties to provide advice, answer questions and educate on the Juvenile Redeploy model;  
  • Works a flexible schedule as assigned by their manager, and can expect that they will work some 10 a.m. until 6 p.m. shifts and some 1 p.m. until 9 p.m. shifts;
  • Participate in regular staffings of Redeploy youth to determine level transitions; address issues; determine eligibility; and completion of services;
  • Participate in regular staffings with the Director of Juvenile Redeploy Services regarding their assigned cases;
  • Participate in wrap-around staffings of Redeploy youth and provide technical assistance to Redeploy sites; develop and author the youth’s success plan;
  • Makes referrals to appropriate services and ensures that the success plan is being followed;
  • Monitoring of level/point system for each youth assigned and ensures that Redeploy agency is using an incentive-based system;
  • Work with the youth’s educational program to ensure participation and success, which will include regular communication with the school;
  • Author the youth’s court report and submit to appropriate parties 3 days in advance of court date;
  • Assess youth’s risk and need using provided instruments when needed;
  • Transmits Redeploy referrals to appropriate Redeploy agencies for intake;
  • Monitors and tracks all Redeploy referrals and maintains up to date information for the Court; the Director of Juvenile Redeploy Services and for evaluation purposes;
  • Transport youth from school or home to the Redeploy sites, and also to appointments;
  • Communicates with Redeploy sites weekly for up to date information on referrals including intake case and active cases;
  • Utilizes case management systems provided by the County and State to include: data input, inputting notes, extracting data and producing reports;
  • Assists Redeploy Project Coordinator with visiting Redeploy sites and evaluations to ensure quality services are being provided;
  • Work with interns assigned to the Redeploy Program;
  • Coordinate the Youth Advisory Board;
  • Meet annual training requirement;
  • Represents the Circuit Court with dignity, integrity, and a spirit of cooperation in all relationships with staff and the public;
  • Can be utilized to fill in for the Project Coordinator- Court Specific position when needed;
  • Performs such other duties as may be assigned by the Director of Juvenile Redeploy Services.
MINIMUM QUALIFICATIONS:
A bachelor’s degree in criminal justice, social work, psychology or related field. Master’s degree preferred but not required. Experience working with the Court or Juvenile Justice field preferred.
Required Knowledge and Skills Knowledge of: and ability to implement
    • Theories, principles practices of the juvenile justice system, including institutional, out-patient and related child development and family dynamics programs.
    • Ability to work with multiple stakeholders in the development and implementation of service delivery programs.
    • Principles and practices of social service delivery, including crisis intervention, particularly in relation to children, youth and families.
    • Computer applications related to the work.
    • Valid driver’s license and working vehicle for travel to multiple job sites
    • Techniques for dealing with a variety of individuals from various socio-economic, ethnic and cultural backgrounds, in person and over the telephone, often where relations may be confrontational or strained.
    • Principles and techniques of making effective oral presentations. Ability to:
    • Work collaboratively as part of a team
    • Perform all duties described above

Physical Requirements:
 
  1. Lift and carry items weighing up to 20 pounds.
  2. Be available to work in-person 100% of the scheduled work week.
  3. Be able to walk for short distances and climb stairs.
  4. Be able to travel throughout Cook County
  5. Ability to intermittently twist to reach equipment and supplies.
  6. Be able to sit at a desk for extended periods of time.
  7. Perform simple grasping and fine manipulation.
  8. Be able to use a telephone and computer keyboard on a daily basis.
